Athiran is set in the 1970s in a secluded mental asylum nestled in the misty high ranges of Kerala. The film follows Dr. Nair (Fahadh Faasil), a psychiatrist who arrives at the mysterious institution to evaluate a peculiar patient: a young woman named Nithya (Sai Pallavi) who is believed to be a "mechanical doll" with no emotions.
